From our experience we know that many people wish to remain at home with their loved ones to spend their last days with their family. It can also be very difficult for family to take care of a loved one who is at the end stages of their life.

Advance End of Life Care Plan

We have a team of home care workers specialising in providing end of life care to customers and support to their families in their own home. Our team will visit you to create an Advanced Care Plan specifically detailing your wishes and the things that are important to you and for your care at this crucial time of your life. The plan will go into detail about what makes your life meaningful, the things you want your family and friends to remember at this time and what is important to you.

Our support can include help with personal care; pain management; medication support, cooking meals and companionship and we can adapt the care plan to the rapidly changing needs of our Customers.

Trusted in Palliative Care

Our team will match a small team of Care Workers that will build meaningful relationships with you and your support network. This ensures continuity allowing us to monitor changes on a daily basis and for you to feel at ease with a regular friendly face.

We also work closely with the local hospice, district nurses and the Continuing Health Care team to ensure our Customers are treated with compassion, dignity and respect at all times.

We look to work together with these professional bodies to provide a continuous service to manage your pain and respect your wishes at this time in your life.

To find out more about our Palliative care services and how we can support you, please contact a member of our team and we will be happy to discuss your needs and see if we can help.

Palliative care is a method used to care for someone who has been diagnosed with a life-threatening illness. It involves recognising and managing pain at the early stages to avoid suffering. Palliative care is more than just managing pain with medication; it can involve emotional support, physical and mental health support, financial support and can include spiritual support if needed.

Palliative Care can be started at any stage once your illness has been diagnosed as life-threatening. This type of care can change quickly to adapt to your changing needs and the level of help required can increase as time goes on. Although it is difficult to say when a person will die, generally a person is considered to be at the end stages of their life when they are expected to die in the next 12 months. Palliative care can last a few days, weeks or sometimes even years depending on your illness and your own personal circumstances.

We understand that not everything can be planned in advance and that your care needs may change rapidly at the end stages of your life. When you are at the end stages of your life we can be available at short notice should the need arise. We work with your family and support network to ensure you are comfortable and your wishes are met and will plan to be there for you as much as you need.

The care plan is your plan so you can make changes as many times as you wish. Simply give us a call and we will come and visit you to discuss the changes. These will then be communicated to the care team and any other professional bodies that provide your care so your choices are known to all the people who provide your care.

Sometimes a member of your family may not understand their illness or be willing to accept help. We can help by slowly introducing care to them at home.

Our goal is to improve the quality of life for our customers and their family so we work closely with the family and friends to ensure that your loved one is receiving the care they need in a dignified way. We focus on your loved one’s values and their own goals of care, forming trusting relationships that grow and allow us to introduce palliative care as needed.

We work with the local hospices, GP, district nurses and family to ensure that joined-up care is central to what we do and all the relevant professional bodies are working together to provide better care.

Testimonials
We are incredibly pleased with Allium Homecare, after all the horror stories I have heard about other agencies. The carers have been a great blessing for my mum. They are always polite and very respectful. Didn’t realise such agencies existed.
